The Patriot, a Choice Community, is a not-for-profit continuing care retirement community comprised of 100-bed skilled nursing and rehabilitation center, a 68-bed Personal Care home and Continuing Care Retirement Community (CCRC) with independent apartment style accommodations in the historic former Somerset Area High School.
Whether you’re looking for a few weeks of therapy following a recent illness or injury, extended nursing care or the peace of mind of living in a personal care setting, we provide compassionate medical services and rehabilitation services in a family-oriented atmosphere.
